FAQ & more

cancellation policy

We ask that patients continue to do their part by cancelling their appointments in the event they are experiencing any cold/flu symptoms or illness. A friendly reminder that we do require 24 hours notice for appointment cancellations. Cancellations made within 24 hours are subject to an out of pocket charge of the total service fee and cannot be billed to your extended health plan. Please respect our time so we can continue to serve our clients in a timely manner, keep wait times down and earn a living.

first-timers

Check your email and complete any intake forms that have been sent. Answer all fields marked as required, including the consent sections. Once completed you will then be prompted to sign the intake using your touch-screen device or click 'Type' to type your electronic signature. Heads up- each designation requires it's own health history intake as per board requirements, so please fill those out to save time and trees prior to your visit.

If you’re having difficulty finding our facility, feel free to give us a call!

direct billing

Yes, we bill directly through your insurance company!

If your insurance requires a prescription and you don’t have one at the time of your appointment, we will have you pay upfront and process your insurance after we receive your doctor's note. Once the claim is processed, the payment will be sent to the insured member rather than the clinic, in order to reimburse you for your appointment.

one-stop-shop

Elevate offers a multitude of products and many different services under one roof!

So save yourself the time and stress, and book your appointments back-to-back with us!

Whether you are thinking of getting a massage, or looking to get a wax or manicure before a big night out, our reception team will be happy to assist you!

booking

Did you know that you can schedule your appointments up to a year in advance with us?

Booking in advance ensures you will get the day and time that suits your schedule best -this is especially helpful for those looking for an afternoon or Saturday appointment since they are in high demand and tend to fill up fast. Closer to the end of the year, you will not be permitted to book all your appointments at once in order to accommodate the clients who receive treatment regularly.

Keep in mind, following a routine is easy for the brain. It is healthy, creates structure in our lives, and allows us to be more efficient with our time!

benefits

In an attempt to utilize the maximum amount of benefits provided by your insurance plan, you should start exercising your coverage before it is too late!

We can't tell your remaining balance nor can we call your provider to find out on your behalf. But if you know how much you are allotted for the year, we can estimate based on your coverage renewal date and how many treatments you have received.

We can also set you up with a treatment plan to ensure you are using up all of your benefits evenly throughout the year.
